Format for U.S. AM Best file

Formats with and without workers' compensation.

AM Best files for the U.S. come in two flavors:

With workers' compensation (CATPREM.DAT)
Without workers' compensation (CATPRMW.DAT)

Each .DAT file:

Has a corresponding .LST file which includes file layout and field descriptions; the .LST file is not required for import.
Includes the following
- Company ID
- Company Name
- AM Best state codes
- 10 lines of business (LOBs)

Touchstone Re remaps both the state codes and the LOBs.
